Apprise

Overview of Service:

Counseling on health insurance matters for older adults. Assistance can be provided with filing medical claims and policy comparisons and can be done via phone or in person by appointment.

Care Management

Overview of Service:

Consumers 60+ who have mental health issues or are developmentally disabled can also receive Care Management services through Mental Health or Intellectual Disabilities.

Congregate Meals

Nutrition, Inc.

Overview of Service:

Provides meals to individuals in group settings. Service includes at least one well balanced meal daily, up to five times a week, containing at least 1/3 of the Recommended Daily Allowances for nutritional standards. There is no fee for this service. Donations are accepted towards the cost of the congregate meals.

Consumer Reimbursement – Pest Control/Fumigation

Overview of Service:

A consumer-directed service which provides reimbursement to individuals for services authorized in their care plan. Consumer reimbursement requires: An approved care plan that outlines what specifically will be reimbursed; submission of receipts for items purchased; completion and submission of a form that details the provision of services (days/hours, service provided and by whom) and sign-offs by the service provider and consumer; and an attestation statement made by the consumer that all submissions for reimbursement are true and correct and reflect that the service was provided as documented in the care plan. Individual is responsible for contacting the Pest Control/Fumigation Provider

Legal

Overview of Service:

Provides education, counseling, and training to individuals on a variety of legal matters that older adults are confronted with in regards to benefits, rights, wills, and taxes. Assistance may be provided by an attorney or non-lawyer. There is no fee associated with this service.

Medical Equipment and Supplies

Overview of Service:

Medical equipment and supplies based on the needs of the consumer and as prescribed by their Primary Care Physician. Must be services not covered by the consumers insurance.

Personal Care/Home Support

Venango Visiting Nurse’s Association

     Contact: 

Overview of Service:

Personal care provides assistance with bathing, dressing, feeding, toileting, ambulating, application and removal of adaptive devices. Medication monitoring and set up.

Home Support assists with daily household activities to help older adults with light cleaning and laundry. Services are based on health and safety needs.

Recovery Specialist

Overview of Service:

A Recovery Specialist helps support a person in substance abuse recovery to overcome barriers to maintaining sobriety.  A Recovery Specialist is person who is currently on a recovery path as well.

Senior Centers

Overview of Service:

Community centers where older adults can get together for social activities, recreation, education, creative arts, physical health programs, and nutritious meals. Venango County Human Services, Older Adult Services (OAS), serves as the front door for the Pennsylvania Department of Aging services at the local level. OAS implements various programs and services offered to older adults in Venango County.
